         Ottawa Carleton Ultimate Association (OCUA)        
             2004 Annual General Meeting (AGM)              


Notice to: OCUA Membership
From: OCUA Secretary

The 2004 OCUA AGM will be held on Thursday, April 15th, 2004 
at 6:30 p.m. in Halls A and B at the Nepean Sportsplex.

At this time, the OCUA membership is invited to submit items 
for inclusion on the 2004 OCUA AGM Agenda.  

Items are required to be submitted to the OCUA Secretary by 
5:00 p.m. on Wednesday, March 31st, 2004.  Submissions shall 
be made electronically or by mail to the following 
coordinates:

Submissions shall include the following information:

1. Description of the item 
  - Description should be no less than 200 words, and no 
    more than 400 words
  - Description should identify the facts related to the 
    item 
2. Identification of the motion for consideration by the 
   OCUA membership
3. Name of the player submitting the item
4. Player's  OCUA Member Identification number (Leaguerunner 
   account number)
5. Telephone and/or e-mail contact information

Submissions shall be submitted complete, and will be 
acknowledged by the Secretary within 5 days of receipt.  
Incomplete or late submissions will not be included on the 
agenda, and items for the agenda will not be accepted at the 
meeting.

The agenda for the 2004 OCUA AGM will be available on the 
OCUA website Thursday, April 8th, 2004.
