Head Coach Application Form

OCUA is looking for 6 Head Coaches to improve the quality of our youth and junior summer leagues. Responsibilities of the Head Coaches will include establishing the structure of the leagues, creating an educational plan to lead the youth and juniors into the best development path possible, and demonstrating the weekly skills and drills with the help of assistant coaches.

Please fill in this form in order to submit an application for one of the Head Coach positions. Application period ends on February 15th, 2013.

Please provide us with your coaching experience and training (NCCP, CPR, First Aid, etc). List any relevant experience or training you might have in Ultimate and in other sports. Include the years, location and level of play.
Please provide us with your ultimate experience as a player. List where, when and what level you played in.
Briefly outline why you want to be a Head Coach for the youth/junior league. Please give us information on what you would do to set the standards of quality for this league. What would you do to help promote Spirit of the Game?
