High School Ultimate Circuit - TEAM registration

To register your team to the 2013-2014 HSUCircuit, you only have to fill in this form once. Once completed register to the tournament(s) you wish to participate in and accumulate points!

Registration to the Circuit is FREE!

If you want to register more than one team from the same school, please fill in one form per team.
Choose all that applies.
Teams need to have a minimum of 10 regular players to be elligible to the Circuit. Please state the names of the players in your team (each team from the same school needs independant valid roster).
